Technical Assistance:
Discussed background checks, TB, sanitizing toys, maintaining documentation when following up with background check inquiries.

Comments:
An unannounced non mandated monitoring inspection was conducted from 3:10m to 4:30pm. The purpose of this inspection was to assess if previous violations were corrected. There were 12 children (31 points) in care of two staff members. The children were observed eating snack, playing with toys, doing an art activity and reading books. A selection of staff, household and children's records were reviewed. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-960-C
Description: Based on observation, a high chair was used for a child under the age of 12 without using the safety waist and crotch straps. Evidence: 1. Child # 2 is 14 months of age. 2. Child # 2 was observed sitting in a high chair without the safety belts being fastened around the waist and crotch area.

Plan of Correction: The child's belt was fastened during the inspection.

Standard #: 22VAC40-111-60-A
Description: Based on record review and staff interview, the provider did not maintain an up to date record for children at the family day home. Evidence: 1. Staff # 2 stated that Child # 1 started care in October. 2. Child # 1's record could not be found during the inspection for review.

Plan of Correction: I have the records and have placed it somewhere. I will find it.

Standard #: 22VAC40-111-90-A
Description: Based on record review and staff interview, the provider did not obtain immunization records for children enrolled. Evidence: Child # 1's immunization records were not available for review during the inspection. Staff # 2 stated that she has the immunization records but could not find them during the inspection.

Plan of Correction: I will find the immunization records and keep with the child's records.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, the family day home provider did not obtain out of state central registry results for an employee who has resided outside of Virginia (VA) in the last 5 years. Evidence: 1. Staff # 1's hire date was 9/10/18. Staff # 1's paperwork indicated that she resided out of VA from 3/2016 through 6/2018. 2. Staff # 2 stated that Staff # 1 mailed the form on 9/14/18. There was no documentation to show if a follow up was conducted to check on the results. 3. The out of state central registry results were not obtained and unavailable for review during the inspection.

Plan of Correction: I will contact the agency to find out what happened to the results.
