54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was completed on 11/26/2018. There were seven children present with the provider and an assistant supervising. The point total was fourteen. Five children's records were reviewed. The provider, household, and assistant's records were all reviewed. The inspector arrived at the provider's home at 10 A.M. and departed at 12:45 P.M. The children were observed during large muscle activities, coloring, during free play time, circle, and during lunch.

Standard #: 22VAC40-111-110-B
Description: Based on review of five children's records, the provider failed to ensure that all physicals and immunizations were signed by the physician or designee. Evidence: 1. Child # 3 immunization record did not have a signature on the form.

Plan of Correction: The provider will obtain the signature ASAP from the physician's office and will monitor all form when they arrive from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on review of five children's records, the provider failed to ensure that all parents were provided in writing the all of the required documents as required by the standard. Evidence: 1. Child # 1 was missing the liability insurance form required by the standard.

Plan of Correction: The provider will have the parent to complete the form today.
