During the inspection, the licensing representative discussed with the provider: - increased water temperature in a bathroom designated for children's use - background and training requirements for potential assistants - updating emergency preparedness plan by 12/12/2018 - age appropriate materials for potentially younger children to be enrolled

An unannounced renewal study was conducted today from 1:00pm-3:45pm. A total of 3 children [7 points] were present with 1 caregiver. Children were observed during transitions from naptime, handwashing, snack and while engaged in play with toys and manipulatives. Nurturing interactions were observed between children in care and provider. A sample of 5 children's records, 4 caregiver/household member records, emergency drills, emergency plan, emergency supplies, physical plant and other documents were reviewed. Standard #: 22VAC40-111-920 Description: Based on observations and interview, the menu for the current week was not dated. Evidence: There were no dates listed on the current menu. Provider informed licensing representative that food substitutions were made that were not documented on the menu posted. Plan of Correction: I will date the menu and list changes made.
Standard #: 22VAC40-111-60-B Description: Based on records reviewed, 2 out of 5 children's records did not include the required information. Evidence: 1. The record for Child #4 was missing the phone number for 1 of 2 required emergency contacts. 2. The record for Child #5 was missing the medical insurance policy number. Plan of Correction: I will get the information from the parents.
Standard #: 22VAC40-111-90-C Description: Based on records reviewed, additional immunizations were not obtained every six months for 1 child under the age of 2 years old. Evidence: The most recent immunization record for Child #1 [21 months old] was obtained 12/11/2017. Plan of Correction: I will ask the parent for the information today.
Standard #: 22VAC40-111-510-A Description: Based on observations, rest mats were not provided to children for resting or napping. Evidence: Upon inspector arrival, 3 children were observed napping on blankets on the floor. Plan of Correction: Children have individual rest mats and I will make sure they are used during naptime.
