An unannounced renewal inspection was conducted beginning at 3:45 and ending at 5:15. There was one child in care, age 7. Reviewed posted information, family and child records. See Violation notice. Home continues to be clean and well kept. There is a good amount of supplies for children of all ages. Provider as documentation of 17 hours of training in addition to Virginia Pre Service. Description: Based on observation, it was determined that the posted menu for breakfast and snack is not dated.

Plan of Correction: The provider will post an up to date menu that is dated every week,

Standard #: 22VAC40-111-60-B
Description: Based on a review of the file for the one enrolled child.,It was determined that the files was missing one element. Evidence: The file does not contain the name and policy number of the medical insurance number.

Plan of Correction: The provider will check with the parent to see if the child has medical insurance. If not, she will note that in the file.

Standard #: 22VAC40-111-180-A
Description: Based on a review of the family records, it was determined that one family member has a TB test that expired. TB tests are to be repeated every two years. Evidence: Resident adult #1 has a Tb test that expired on 9/17/17.

Plan of Correction: The family member is trying to get a copy of his form from the clinic. The form will be put in his file as soon as it s received.
