- Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the new address of the Family Day Home (FDH). Documentation was obtained from the provider to reflect the zoning capacity for this new address. In addition, the Emergency Preparedness and Response Plan for the new address was also reviewed. The new fingerprint checks for all household members and staff were completed and reviewed.
The following inspection took place between the hours of 3 pm through 4:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 4 children (4 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: drop-off, snack, games, clean up, bathroom break, homework time, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 2 of 5 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (12/11/2018), the following children were without a physical: Child #1, and Child #5. 2. The start date for Child #1 was listed as 09/04/2018. The start date for Child #5 was listed as 10/13/2018. Plan of Correction: The parents of both children shall be contacted and the missing documentation shall be obtained and placed on file.
Standard #: 22VAC40-111-60-B Description: Based on review, 2 of 5 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (12/11/2018), the policy number was not listed for the medical insurance of Child #4, and Child #5. 2. While the policy name was listed for both children, the space for the policy number was left blank. Plan of Correction: This missing information shall be obtained and placed on file.
Standard #: 22VAC40-111-90-A Description: Based on review, 2 of 5 children's files reviewed indicated that the Provider did not obtain documentation that all children have been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) prior to attending the Family Day Home (FDH). Evidence - 1. On the date of inspection (12/11/2018), the following children were without a immunization page: Child #3, and Child #5. 2. The start date for Child #3 was listed as 11/13/2018. The start date for Child #5 was listed as 10/13/2018. Plan of Correction: The parents of both children shall be contacted and the missing documentation shall be obtained and placed on file.
