Technical Assistance:
The Provider requested information for training resources. Information was provided regarding web sites and resources from the public library. The Provider reported that she will complete the balance of required training hours by the end of the year.

An unannounced renewal inspection was conducted in the Family Day Home on December 6, 2018. The Provider and 4 children were present. The ages of the children ranged from 9 months to 4 years. Based on their ages, the points totaled 10. The children were observed during free play, the lunch meal, and nap time. The Provider was attentive to the children's needs. Age appropriate toys were available to the children. Reportedly, there are 7 children currently enrolled. The Provider reported that medications are not administered as stated in the written medication policy. The Provider's record was reviewed. Documentation of the practiced emergency drills, the emergency contact person, and the revised emergency preparedness plan were also reviewed. A sample of 4 children's records was reviewed. One of the four children arrived to the home after lunch and the parent reported that the child had eaten prior to arrival. The Provider served lunch to three children, the foods served were listed on the posted menu. Findings were reviewed with the Provider at the exit. Description: Based on reviewed documentation the Provider failed to obtain a signed copy of the physical examination and immunizations record which were signed by a licensed physician, the physician's designee, or an official of a local health department. Evidence: The record of child 4. contained a printed copy of the child's current medical information, the documents were not signed by the physician or his designee.

Plan of Correction: The parent was notified at the time of the inspection and will contact the child's physician to request a signed copy for record retention.

Description: Based on a sample review of children's records, the Provider failed to provide in writing to the parents the following information as required by the Standards: general daily schedule that is appropriate for the age of the enrolling child and policies for termination of care. Evidence: A sample of 4 children's records was reviewed. The Provider had not completed the information on the model form that addresses policies for termination of care and the daily schedule in each of the 4 records.

Plan of Correction: The Provider will complete the information on the model form and will provide a copy to each parent. Each parent will be requested to review, initial and date.

Standard #: 22VAC40-111-140-B-3
Description: Based on interview and reviewed documentation, the Provider's CPR certification was not current. Evidence: Documentation of the CPR certification expired August, 2018.

Plan of Correction: The Provider will update the certification by completing a CPR course.

Standard #: 22VAC40-111-140-B-4
Description: Based on interview and reviewed documentation, the Provider's first aid certification has expired. Evidence: Documentation of the first aid certification expired in August, 2018.

Plan of Correction: The Provider will schedule and attend a first aid training to update the certification.
