63.2 Licensure and Registration Procedures
- Technical Assistance:
Inspector and provider discussed infants play time and background checks.
An unannounced monitoring inspection was conducted today from 10:50am-12:40pm. There were 3 children (9 points) directly supervised by 1 staff. The home, 2 staff records, 2 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed watching television, playing freely and eating lunch. Lunch is provided by the families. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-770 Description: Based on observation, the provider did not have a working battery-operated weather band radio available. Evidence: The provider could not find the battery-operated weather band radio at the time of inspection. Plan of Correction: I will find my battery operated radio.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination within 30 days attendance. Evidence: Child 2 does not have documentation of a physical examination within 30 days of attendance. (Child B, start date 9/2018) Plan of Correction: I will give the forms to the parents.
Standard #: 22VAC40-111-70-A Description: Based on record review, the parents were not provided in writing all required information prior to the child's first date of attendance. Evidence: Child 2 does not have the information to parents record which includes but is not limited to information regarding holidays, check in and out procedures, termination policies, payment policies. Plan of Correction: I will get the blank one and get it to the parents.
Standard #: 22VAC40-111-600-B Description: Based on observation, an infant spent more than 30 minutes of consecutive time during waking hours in a confining piece of equipment. Evidence: At about 11:15am, Child 2 woke up from napping while in a swing. At 11:55am, Child 2 was removed from the swing and the child's diaper was changed. After the diaper changed, Child 2 was placed in a crib. At 12:10pm, the child was removed from the crib to be bottle fed. Child 2 spent about 50 minutes combined strapped into a swing or placed in a crib awake. Plan of Correction: The child was removed from the crib. I will make sure to get a cloth and set toys down to play.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider did not ensure child's hands were washed with liquid soap and warm running water when the child's hands were dirty. Evidence: After eating lunch, Child 1 was removed from the high chair. The provider wiped the child's hands with a dry paper towel and was placed into a crib for nap time. The child's hands were dirty from lunch and from the mucus from their nose. The child's hands were not washed. Plan of Correction: The provider washed the child's hands when notified of the violation. Provider stated they will make sure to wash hands.
Standard #: 63.2-1720.1-B-2 Description: Based on observation, all staff members did not submit to fingerprinting. Evidence: Staff 1 and 2 did not submit to fingerprinting. No results were found in their record. Upon interview, the provider stated no fingeprinting was conducted. Plan of Correction: We will get the fingerprinting within 10 days.
Standard #: 63.2-1721.1-B-2 Description: Based on observation, all household members did not submit to fingerprinting. Evidence: Staff 1, 2, 3, 4, 5 did not submit to fingerprinting. No results were found in their record. Upon interview, the provider stated no fingeprinting was conducted for any household members. Plan of Correction: We will get the fingerprinting within 10 days.
