An unannounced renewal inspection was conducted on December 17 2018 between 11:30am and 1:00pm. There were 4 children in the direct care of the provider and an assistant. Children were observed eating lunch, being diapered, and taking a nap. The home is clean, organized, and offers plenty of materials for the children to use. Standard #: 22VAC40-111-100-A Description: Based on record review, 1 of 4 children's records reviewed did not have documentation of a physical examination by or under the direction of a physician within 30 days after the first date of attendance. Evidence: Child #4 record does not have documentation Plan of Correction: I will have the parent get the forms by the pediatrician and will have it in the file by the date specified.
Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 2 household members and 1 of 2 staffing records do not have documentation of a current Report of Tuberculosis Screening or test every two years from the date of the first screening. Evidence: Household member #1 record last documented report is dated 05/31/2018. Staff member #2 does not have documentation of a tuberculosis screening. Plan of Correction: I will get the TB forms in their files by January 4th 2019
