63.2 Facilities and Programs..
An unannounced complaint investigation was conducted on 8/8/2018 and 8/29/2018 in response to a complaint made to our office alleging inadequate staffing and supervision of children in care. Based on interviews conducted and records review, there is a preponderance of evidence to support the claim; therefore, the complaint is deemed valid. Areas of noncompliance are documented on the violation notice. An additional violation, not related to the complaint, has also been cited. Standard #: 22VAC40-111-570-A Complaint related: No Description: Based on observations, review of records and interviews conducted, it was determined that the provider did not ensure that the points per caregiver did not exceed 16 points. Evidence: 1. On 7/25/18 at approximately 9:33 am, the provider was observed alone with 11 children in the home. 9 children were enrolled and in the care of the provider and the two additional children were the provider?s own children that lived in the home. 2. On 7/25/18 at approximately 9:33 am the total number of points for the 11 children was 25 points with one caregiver. Two caregivers were required. 3. The provider verified that on 7/25/18 she was alone supervising 11 children with a total of 25 points for approximately 5-10 minutes until the second caregiver (Assistant #1) arrived at the family day home .
