Comments:
An unannounced monitoring visit was conducted today between the hours of 9:00am through 10:30am with the provider on site. There were 3 children (5 points) with the provider. A sample of 2 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, playing games, writing on paper and reading books with the provider. Interactions between the children and provider were positive. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain from each adult household member a current report of tuberculosis screening form every two years from the date of the first screening. Evidence: 1. Household Member #1 and Household Member #2 had a report of tuberculosis screening form dated 10/21/2016 and was more than two years and is required to have an updated screening.

Plan of Correction: I will have them set up an appointment.

Standard #: 22VAC40-111-320-G
Description: Based on record review, the wood burning fireplace had not been inspected annually. Evidence: 1. The provider had the wood burning fireplace last inspected on 11/22/2017 and needs to have an updated inspection.

Plan of Correction: I will set up an appointment to have it inspected.
