The Inspector was on site today after receiving self-reported information from the provider, regarding administrative practices and the care of children. No violations associated with that report were cited today. However, violations associated with the monitoring inspection portion of today's visit were cited. Children's records, administrative records, staffing practices, the physical plant, children's nutrition, caregiver records, training, and programming were inspected today. This inspection was completed in the middle of the day. All children were observed finishing lunch, playing with toys, assembling puzzles, and preparing for their afternoon nap. The children were served macaroni with tomato sauce, plums, and milk for lunch today. This inspection was completed between approximately 11:45am and 1:00pm. Thank you for meeting with me today. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record A does not include documentation showing the child's last day of attendance. 2. Child record C does not include the parent's medical insurance policy ID number.

Plan of Correction: Both records will be updated.

Standard #: 22VAC40-111-80-A
Description: Based on a review of children's records, it was determined that one record does not include documentation of the child's proof of identification. Evidence: Child record B does not include documentation to show that the provider has viewed the child's original proof of identification.

Plan of Correction: The file will be updated.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations, it was determined that the provider did not have the findings from the most recent inspection clearly posted. Evidence: Upon arrival, the Violation Notice from the most recent inspection was hanging beneath other documents from the same inspection. The Violation Notice was not clearly visible.
