Comments:
An unannounced monitoring inspection was conducted today from 9:15am-10:15am. There were 4 children (11 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and coloring.There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not ensure parents were notified in writing of all required information. Evidence: Child 1 does not have information regarding the provider's medication policy, check in and out procedures, termination policies and operating hours.

Plan of Correction: The child is a family member and we will have the paperwork completed.
