54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted beginning at 9:15 a.m. and ending at 11:00 a.m. with the provider and assistant. There were ten children in care ranging in age from seven months to four years of age for a total of twenty-six points. A toddler was napping during the inspection. Infants were observed having play time on the floor and in infant equipment (infant jumper). Older children were observed having story time. After story time, children prepared for an additional morning snack of vanilla wafers and water. After snack, preschool children participated in worksheets for today's lesson and younger children had play time. An infant was observed being fed. Four children's records, three caregiver records and one household member record were reviewed. Emergency drills were up to date. Three pet records (cat) were reviewed. The weekly menu was posted. Standard #: 22VAC40-191-60-B Description: Based on review of staff records, the provider failed to obtain a completed sworn statement or affirmation by the first day of employment. Evidence: Staff #3, start date 8/13/2018, did not have documentation of a sworn statement in the record. Plan of Correction: Staff #3 will complete the sworn statement today.
