An unannounced renewal inspection was conducted today from 10:40am-12:30pm. At the beginning of the inspection there were 7 children (20 points) with an assistant, the provider arrived approximately 15 minutes later. A total of 8 children (24 points) and two caregivers were observed during the inspection. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, painting, eating lunch, and transitioning to nap time. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not maintain complete children's records. Evidence: Child#1's record is missing the authorization for emergency medical care. Child#4's record is missing the address of both emergency contacts. Child#5's record is missing the address of both emergency contacts, and the child's medical insurance policy number. Plan of Correction: All records obtained.
Standard #: 22VAC40-111-80-A Description: Based on record review, the family day home did not have documentation that they had viewed the child's proof of identity within seven days of the child's first day of attendance at the family day home. Evidence: The records of child#1 (DOE: 4/28/2018), #2 (DOE:2/5/2018), #4 (7/23/2018) and #5 (1/2/2019) do not contain documentation that the provider verified each child's proof of identity within seven business days of the children's first day of attendance. Plan of Correction: All documents reviewed and fixed.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation to verify that a child has been adequately immunized according to the requirements of the Code of Virginia and applicable State Board Health. Evidence: Child#5's record does not contain the child's immunization record. Plan of Correction: Record obtained.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain documentation of additional immunizations once every six months for a child under the age of two years. Evidence: Child#1's record has an immunization record on file that was last updated on 03/28/2018, the child was 14 months of age when their immunization record was last updated. Plan of Correction: Received immunization records.
Standard #: 22VAC40-111-240-A Description: Based on record review, ares of the home were not maintained in a safe a operable condition. Evidence: In the infant room a crib was observed missing a screw on the upper left corner. In the main programming area a small black chair was observed with a tear about 5 inches wide in the fabric exposing the inside foam. Plan of Correction: Screwed replaced in crib. Chair tossed.
Standard #: 22VAC40-111-570-A Description: Based on observation, record review and interview, the Family Day Home exceeded the 16 points allowed per caregiver. Evidence: There were 7 children present totaling 20 points at the time the inspector arrived (10:40 am) with only one caregiver present. The provider arrived approximately 15 minutes after the inspector arrived. Each caregiver cannot exceed 16 points, the one caregiver was over by 4 points. Plan of Correction: There was a school delay and staff#2 was late while provider was at bus stop. Will not happen again.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review and interview with the provider, the provider did not obtain a central registry report within 30 days of a household member becoming 14 years old. Evidence: Household#1's file does not contain a central registry check report. Plan of Correction: All CPS checks filed.
Standard #: 63.2-1721.1-A Description: Based on record review and interview, it was determined that a required central registry report was not updated. Evidence: Staff#1's most recent central registry check was dated 1/15/2014. The document expired on 1/14/2019 and an updated central registry check should have been completed. Plan of Correction: Updated.
