Technical Assistance:
**Discussed with provider that the current Emergency Preparedness plan requires a review by February 28, 2019. **The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated Januaruy 17, 2019.

Comments:
An unannounced monitoring inspection was conducted today from 9:10am-11:10am. There were 10 children (30 points) directly supervised by the provider and 2 staff. The home, 4 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today in accordance with the provider's policy. Children were observed watching a children's program, eating breakfast, playing with puzzles and napping. There was an abundant supply of books, toys, and materials for the children. An exit interview was conducted with the provider on the date of inspection and the provider has requested time to compose a plan of correction. A plan of correction must be submitted to the licensing inspector via email within 5 days of the date of inspection for the correction actions to display on the online compliance history. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-B
Description: Based on record review and provider statement, shelter-in-place drill were not practiced twice per year. Evidence: One shelter-in-place drill was conducted in the past year.

Plan of Correction: Shelter-in-place drills will be conducted twice per year.

Standard #: 22VAC40-111-890
Description: Based on on observation and provider statement, food brought from home were not labeled in a way that identifies the owner. Evidence: Infant food provided by the parent was not labeled in a way that identifies the owner.

Plan of Correction: All food will be labeled going forward.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for one of five children's records reviewed. Evidence: Child 4's record did not contain a record of a physical examination.

Plan of Correction: Child 4's parents were notified and reminded to provider result of physical examination.

Standard #: 22VAC40-111-120-B
Description: Based on record review, three of three employee records were incomplete. Evidence: 1. Adults 1 and 3's records contain one of the two required reference checks. 2. Adult 5's record does not contain documentation of any references checked.

Plan of Correction: Will ask staff to provide additional references.

Standard #: 22VAC40-111-530-A
Description: Based on observation, children didn't have linens consisting of a tops and bottom cover. Evidence: One child was observed sleeping on a mat without a bottom cover.

Plan of Correction: Will ensure mats have covers.

Standard #: 22VAC40-111-540-A
Description: Based on observation, safety straps were not used for one child occupying a high chair. Evidence: One child was observed seated in a highchair without the safety belt fastened.

Plan of Correction: Will remind employees to fasten safety straps on highchairs.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, three of three staff records do not contain the results of a national criminal background check. Evidence: Adults 1, 3, and 5 do not have the results of a national criminal record check.

Plan of Correction: Results for adults will be printed and filed.

Standard #: 63.2-1721.1-B-2
Description: Based on record review, one household member and one applicant/agent did not have record of a satisfactory completion of a fingerprint background check prior to the required completion date of 10/1/2018. Evidence: Adults 2 and 4 do not have the results of a national criminal background check on file at the family day home.

Plan of Correction: Results for adults will be printed and filed.
