Comments:
An unannounced renewal study was conducted today, There are 5 children present (10 points) with one caregiver. Three adult and 5 children's records were reviewed. See Violation notice for missing items. Children were observed resting quietly, washing hands, eating snack and doing an educational project. Emergency escape drills, shelter-in-place drills, first aid supplies, dog's rabies vaccinations and emergency supplies were all in compliance. Standard #: 22VAC40-111-800-A
Description: Based on a review of the written emergency preparedness plan, the plan has not been reviewed and updated annually. Evidence: The plan available for review is dated 9/5/16.

Plan of Correction: The plan has been reviewed and updated. I just didn't sign the form.

Standard #: 22VAC40-111-120-B
Description: Based on a review of records, one staff record is incomplete. Evidence: Adult B's record does not contain documentation of 2 references.

Plan of Correction: References were done verbally. I will document them in the record.

Standard #: 22VAC40-111-180-A
Description: Based on a review of records, the caregiver and adult household member did not obtain subsequent TB screenings at 2 year intervals. Evidence: Provider's TB test results are date 6/22/16 - expired. Adult Household Member (HHM) A's- TB results are dated 6/22/16 - expired.

Plan of Correction: We will get new TB tests screening.

Standard #: 63.2-1720.1-B-2
Description: Based on a review of records, one assistant did not have a completed fingerprint result on file prior to employment. Evidence: Adult B (Date of Hire: 8/2/18) has fingerprint results on file dated 8/29/2019.

Plan of Correction: I will not hire anyone until the fingerprint results are returned.

Standard #: 63.2-1721.1-C
Description: Based on a review of records, one household member has a disqualifying background search. Evidence: HHM -A's Background search lists them as not eligible.

Plan of Correction: A waiver is being applied for at this time.
