Comments:
The licensing inspector conducted an unannounced monitoring inspection on 1/29/2019 from 12:42pm to 3:20pm. The children were playing and napping upon the inspector?s arrival, and they were later observed playing with toys, completing puzzles and reading books. Today, there were four children, totaling 9 points, present with one caregiver. The caregiver was observed interacting with the children. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children. The menu was posted. The provider does not administer medication at this time. The provider does not provide transportation. The provider has pets. Five children?s records, one caregiver record, and one household members record was reviewed during the inspection. Last emergency drill: 1/22/2019 Last shelter-in-place drill: 1/22/2019 The violations from the previous inspections were checked for corrections. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, the provider did not ensure that two of five children's records obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record of child #3 (start date: 8/20/18) did not have documentation of a physical record. 2. The record of child #4 (start date: 1/22/16) did not have documentation of a physical record.

Plan of Correction: I will ask the parents about the physicals today and get them as soon as possible.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure that the outside areas of the family day home were maintained in a clean, safe, and operable condition. Evidence: The providers backyard had protruding nails on the fence. The head of the protruding nails were exposed.

Plan of Correction: I will pull out the nails before the kids go back outside.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that child-resistant protective covers larger than 1-1/4 inches in diameter were installed on all unused electrical outlets accessible to children under five years of age. Evidence: The provider had uncovered electrical outlets in the kitchen that the children eat lunch in.

Plan of Correction: I have got outlet covers and will put them back on.
