Technical Assistance:
Discussed diapering surfaces, bottle and food labeling, and placement of items and children in cribs.

Comments:
An unannounced monitoring inspection was conducted today from 11:15am-2:15pm. There were 9 children (25 points) supervised by 2 direct care staff. The home, 2 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely and napping. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on observation, the provider did not maintain a first aid kit that contained all of the required items. Evidence: The first aid kit did not contain a first aid instruction manual.

Plan of Correction: I will purchase one today.

Standard #: 22VAC40-111-830-B
Description: Based on record review, the provider did not maintain documentation of shelter-in-place drills practiced twice per year. Evidence: There was no documentation of shelter-in-place drills completed for 2018.

Plan of Correction: I will complete shelter-in-place drills this year.

Standard #: 22VAC40-111-120-B
Description: Based on record review, 1 of 2 caregiver records did not contain documentation of all required information. Evidence: Caregiver B's (date of hire: 1/28/2019) record did not contain documentation of emergency contact information, two completed reference checks, completed orientation and training, and proof of education and/or training.

Child #3's (start date: 10/29/2017) record did not contain documentation of the father's work telephone number, a second emergency contact person, a completed physical signed by a physician, and parental acknowledgement of the medication authorization policy and emergency preparedness plan. 3. Child #6's (start date: 4/15/2017) record did not contain documentation of written authorization for emergency medical care. 4. Child #8's (start date: 8/6/2018) record did not contain documentation of written information for parents and parental acknowledgement of the emergency preparedness plan. 5. Child #9's (start date: 8/9/2018) record did not contain documentation the father's work telephone number, written information for parents, and parental acknowledgement of the liability insurance.

Plan of Correction: I will print all missing paperwork and place in children's files.

Description: Based on observation, areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition. Evidence: 2 out of 3 infant cribs had chipped paint and deteriorating wood on the edges.

Plan of Correction: I will purchase plastic covers for edges of cribs.

Description: Based on observation, the provider did not ensure a landline telephone is available, operable, and accessible during the family day home's hours of operation.

Plan of Correction: It was ordered today.

Description: Based on record review, 1 of 2 caregiver records did not contain documentation of a completed sworn statement or affirmation. Evidence: Caregiver B's (date of hire: 1/29/2018) record did not contain documentation of a completed sworn statement or affirmation.

Description: Based on record review, 1 of 2 caregiver records did not contain documentation of completed fingerprint results. Evidence: Caregiver B's (date of hire: 1/28/2019) record did not contain documentation of completed fingerprint results.

Plan of Correction: Caregiver went to appointment 1/18/19 and is awaiting results.
