An unannounced complaint inspection was conducted on 01/22/2024 in response to a complaint received by the licensing office on 01/10/2024 in the areas of staffing, background checks and children?s records. There was a total of 49 children, ages 2 months-13 years and 9 staff present during the inspection. A total of 5 child?s record and 5 staff records were reviewed during the inspection. The inspector arrived at 2:15 pm and departed at 3:50 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
The complaint was determined to be valid.

Based on record review, the center failed to obtain a copy of the child abuse and neglect registry check from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 3 contained evidence that Staff 3 had resided outside of the state of Virginia within the preceding five years. The record for Staff 3 contained a documented hire date of 09/28/2023. There was not a child abuse and neglect registry check from all of the states in which the individual had resided in the preceding five years available for review during the 01/22/2024 inspection. Staff 3 was observed working during the 01/22/2024 inspection.Plan of Correction: Have reached out to the state and now understand the process. The request will be made.

Based on record review, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation and to obtain the findings from a search of the central registry within 30 days of employment.
Evidence:
1. The record for Staff 2 contained a documented hire date of 10/19/2023. There were no findings from a search of the central registry for Staff 2 available for review during the 01/22/2024 inspection. Staff 2 was observed working during the inspection.
2. The record for Staff 3 contained a documented hire date of 09/23/2023. There were no findings from a search of the central registry for Staff 3 available for review during the 01/22/2024 inspection. Staff 3 was observed working during the inspection.Plan of Correction: Have created a system to ensure that all of the information is in the record before being filed. Will follow-up on searches when they are not returned.
