An unannounced monitoring inspection was completed on 2/1/19. There were 11 children present with the provider and an assistant supervising. The point total was 25. Eleven children's records, the provider's record, a household member's record, and two caregiver records were reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 12:20 pm and departed at 4:00 pm. The children were observed during nap, during afternoon snack, during the diapering process, and during departures. There was discussion with the provider about the following: hand washing practices after eating, post office box addresses documented for one emergency contact (child #4), work phone numbers other than cell phones for parents,

Standard #: 22VAC40-111-110-B
Description: Based on record review, the provider failed to ensure that each health report was signed by a licensed physician, the physician's designee, or an official of a local health department. Evidence: 1. The immunization record for child #8 dated 1/15/19 was not signed.

Plan of Correction: The provider will notify the parent today. The provider will have the parent provide a signed immunization record for child #8.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the center failed to ensure that there was written acknowledgement by the parent of receipt of the information required by this section. Evidence: 1. The model form "Information for Parents" for children #8,9, and #10 did not have a parent signature to acknowledge receipt of the information on this form required to be disclosed to the parent. The first page of this form was in each child's record; however, the second page that includes the parent's signature was missing from each child's file (#8, #9, and #10).

Plan of Correction: The provider will have each of the parents sign the second page.
