63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was conducted on 02/06/2019 from 11:30 am to 1:30 pm to review licensing health and safety standards. At the time of the inspection there was one child in the care of one caregiver. The sample size consisted of five children's records and a review of three adult records. The child and provider were observed during hand washing, lunch, free play, story time, transitions, behavioral management. Nap time procedures were discussed. Standard #: 22VAC40-80-120-E-1
Description: Based on observation and interview the provider failed to post the most recent copy of her license. Evidence: 1. During the inspection the inspector was unable to locate a posted copy of the most recent license. 2. The provider stated that the license is normally posted in the same location at all times, however during the inspection she was unable to find it.

Plan of Correction: The provider requested the inspector to forward another copy of the license to post.

Standard #: 63.2-1721.1-B-2
Description: Based on record review and interview the provider failed to provide documentation of a completed fingerprint background check. Evidence: 1. Three adult household records reviewed. The record for adult #2 did not contain documentation of a completed fingerprint check. 2. The provider stated that the fingerprint check was completed, however she was unable to find the results during the inspection.

Plan of Correction: The provider will follow up with Office of Bureau Investigations to see if a copy of the results can be obtained and if not will reschedule appointment for fingerprinting. The provider will keep the licensing inspector informed of the progress.
