Technical Assistance:
Technical assistance was provided for MAT forms and diapering.

Comments:
An unannounced renewal inspection was conducted from 10:25am to 11:45am. There were 9 children (32 points) in care of 3 caregivers. The children were observed playing with toys, listening to music, reading books and doing circle time. 5 children's records, 2 household member and 3 caregiver records were reviewed. Training records were reviewed. Emergency drill records were current. A walkthrough of the family day home was conducted. Areas of non-compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's name full name and date. Evidence: 1. There were 5 infants in care. 2. There were two bottles that were labeled with the child's name but were not dated. 3. There were two additional bottles with milk that were not labeled and dated.

Plan of Correction: I will check to make sure that bottles are labeled with names and dates.

Standard #: 22VAC40-111-320-A
Description: Base on observation, small electrical appliances were not unplugged when not in use. Evidence: Two small electric heaters were found to be plugged in the rooms adjacent to the day care area. An electric kettle and a toaster oven were not unplugged in the kitchen and were not being used.

Plan of Correction: The appliances were unplugged during the inspection.
