Comments:
An unannounced Code Compliance inspection was conducted with the program?s Principal on 2/22/24 from 9:30am to 11:15am. There were 10 children present, supervised by one staff. Children were observed playing in centers, cleaning up the classroom, and eating a snack from home. Required postings were observed; four child records and three staff records were reviewed for required documentation. The program met the requirement for staff on-site with current CPR/First Aid training. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 3/8/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.031-B-1
Description: Based on observation, the religious exempt program did not implement procedures for hand washing by staff and children before eating. Evidence: On 2/22/24, at approximately 9:45am, the children transitioned from playing to cleaning up the classroom and then snack and did not wash their hands before they began eating their snack.

Plan of Correction: Review hand washing regulations with staff and posted signage.

Standard #: 22.1-289.031-B-2
Description: Based on observation, the religious exempt program did not implement procedures for appropriate supervision of all children in care. Evidence: On 2/22/24, from approximately 9:32am to 9:34am, a four-year-old child was observed walking out of the classroom, down the hall and into the restroom, and back to the classroom, without supervision. The teacher for the class was later observed leaving the classroom to look for another teacher, leaving the children in the classroom unsupervised for less than one minute.

Plan of Correction: Our policy is for the class, as a whole, to have bathroom breaks. This was reinforced with the teacher.

Standard #: 22.1-289.031-B-4
Description: Based on review of four child records, the religious exempt program did not implement procedures to ensure that all children in the center were in compliance with immunization provisions. Evidence: The record for Child D did not contain documentation of immunizations.

Plan of Correction: Followed up with office staff, notified parents about missing records.

Standard #: 22.1-289.031-B-5
Description: Based on observation, the religious exempt program did not implement procedures to ensure that all areas of the premises accessible to children were free of obvious injury hazards. Evidence: 1. Two electrical outlets were observed to be uncovered on 2/22/24, in a classroom of children who were four- to five-years-old. 2. A pile of approximately three or four rugs was observed in the classroom, and posed a tripping hazard.

Plan of Correction: This policy was reinforced with the teacher. The hazard items were rectified at once.

Standard #: 8VAC20-770-40-D-2
Description: Based on review of three staff records, the religious exempt program did not ensure that staff submitted a signed sworn disclosure statement prior to the first day of employment. Evidence: There were no sworn disclosure statements on record for Staff #1, with an employment date of 8/1/14; Staff #2, with an employment date of 7/1/19; or Staff #3, with an employment date of 8/16/18.

Plan of Correction: Sworn disclosure statements are now on file. Pre-K was started after these employees were hired and was not required.
