Comments:
An unannounced Monitoring inspection was conducted this date. A total of 9 children (25pts) with Provider and 1 assistant. When the inspector arrived this morning the children were observed at the table top, coloring with colored pencils, writing their alphabet with assistance of staff. The younger children were playing freely on the floor with equipment and toys that were determined age appropriate. A nice transition was observed from the table top to the floor where the children were observed playing with wooden puzzles, kitchen set and books. Staff were observed changing infants' positions frequently to encourage stimulation and attention. Diapering and hand washing procedures were observed. A total of 5 children's records and all staff and household records were reviewed. Previous violations from the July licensing inspection were reviewed. Corrective actions were verified with all but one repeated issue regarding health exams. This inspection began at 9:40am and ended at 11:50am. Thank you for your cooperation today. Standard #: 22VAC40-111-960-E
Description: Based on observation of the refrigerator, one bottle was not labeled. Evidence: One baby bottle located in the refrigerator was not labeled with the child's name and date.

Plan of Correction: We will ensure that all bottles will be labeled with the child's name when parents bring them. If they are not labeled when they arrive, we will label and date them. The bottle was labeled immediately.

Standard #: 22VAC40-111-100-A
Description: Based on child record review, Provider did not obtain a physical exam within 30 days of the first day of attendance. Evidence: Child #3 began care on 1/2/19, no health exam was found on file. Child #4 began care on 3/1/18, the health exam found in the record is dated 7/23/18.

Plan of Correction: Parents will be informed that within 30 days a health exam is required to continue with care.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, authorization for emergency medical care was not obtained for one out of 5 records reviewed. Evidence: Child #4's record does not contain parent authorization for emergency medical care.

Plan of Correction: I will have this parent sign the authorization today.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunizations were not obtained for one child prior to beginning care. Evidence: Child #4 began care on 3/1/18, immunization documentation is dated 8/15/18.

Plan of Correction: No child will began care without providing immunization documentation.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, updated immunizations were not obtained every six months for one child under the age of 2. Evidence: Child #1 is under the age of 2, the last documented immunizations are dated 7/24/18. Updated immunizations for children under the age of 2 must be obtained every six months.

Plan of Correction: I will inform all parents of children under the age of 2 to provide updated immunizations at least every six months.

Standard #: 22VAC40-111-530-B
Description: Based on observation of the cribs on site, a tight fitting bottom cover was not maintained. Evidence: All five cribs on site had sheets that were not tight fitting.

Plan of Correction: Will replace these sheets with smaller ones or alter the sheets to make them tighter
