Annual training will be reviewed at the next licensing inspection.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 11 children present (24pts) with Provider and 1 assistant. One child is Provider's child and not counted in licensed capacity. When the inspector arrived, the children were seated at the tables having lunch. Lunch today was chicken, rice and soup. Ample supply for second helpings if requested. Walk through of the program area was made. Some painted artwork and the letter "E" was painted and on display. Staff were observed sitting with children, assisting and encouraging them to eat. After lunch, a nice transition to nap time was observed. Washing and sanitizing of the table tops was completed after lunch. A total of 9 children's records were reviewed, all staff and 1 household record were also reviewed. One violation found regarding annual review of emergency contacts. Previous violations reviewed from the October licensing inspection were reviewed and corrective actions were verified. This inspection began at 11:45am and ended at 1:20pm. Description: Based on child record review, 3 out of 9 records reviewed did not have an annual review of documentation and a signed acknowledgment of the review by the parents. Evidence: Child #1,3 and #4 did not have an annual review of emergency contacts. Child #1's last review was 1/17, Child #3's last review was 1/18 and Child #4's last review was 1/18.

Plan of Correction: I will have parents review these today.
