A unannounced monitoring inspection was conducted with the provider and one assistant. Nine children were present and the age range was 14 months to five years old. Children were observed during snack time, singing in Spanish and English also learning letters and colors. One medication was reviewed . Three house hold members the provider and one assistant records were reviewed. Standard #: 22VAC40-111-60-A Description: Based on review of five children's records not all were complete. Evidence: Child A with a start date of 9/3/18 did not have the phone number for one emergency contact. Child B, with a start date of 8/23/18 was missing Mother and Father's work address and missing two emergency contacts. Child C, with a start date of 1/18/19 was missing one emergency contact. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-111-240-B Description: Based on observation the provider did not maintain the play ground area. Evidence: There is weed block sticking up at the ends of slides and all the way around the perimeter of the playground equipment. Posing a tripping hazard to the children. Plan of Correction: The play ground equipment will be off limits until fixed.
