Today an unannounced Renewal inspection took place between 10:10am and 12:00pm. There were 5 children present with the provider for a total of 12 points within the supervision guidelines. The children were observed playing freely, practicing their letters, numbers and colors and playing with building blocks. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-760-A-1
Description: Based on observation and interview, the first aid kit did not contain all required items. Evidence: The first aid kit only contained one triangular bandage or antiseptic cleaner. The provider stated that she didn't know she needed two triangular bandages.

Plan of Correction: I will get these tomorrow.

Standard #: 22VAC40-111-980-A
Description: Based on interview, a general written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home. Evidence: At the most recent inspection, the provider stated that she takes the children to the playground down the street to play outside; the provider received a violation for not obtaining written permission from the children's parents to leave the family day home. During today's inspection the provider stated that she still takes the children to the playground if the weather is nice. When asked about the permission forms from the parents, the provider stated that she has not yet obtained written permission forms from the parents.

Plan of Correction: I will take the model form from the DSS website and have the parents fill it out.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 and Child #3's files do not contain the families medical insurance policy numbers. Child #5's file does not contain the name of the families medical insurance.

Plan of Correction: I will get those today.

Standard #: 22VAC40-111-320-B
Description: Based on observation, a surge protector accessible to children under five years of age did not have child-resistant protective covers. Evidence: There is a dresser pulled approximately 18 inches away from the wall in the basement of the family day home. There is a long surge protector with one unused electrical outlet. The outlet does not have a protective cover.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed with liquid soap and warm running water. Evidence: During the inspection the children asked to eat a snack. The provider was observed squirting hand sanitizer on the children's hands prior to eating.

Plan of Correction: I will start doing it today.

Standard #: 22VAC40-665-400-C
Description: Based on observation and interview, 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center were not posted in a visible and conspicuous place. Evidence: The number for the local police, fire department or poison control is not posted anywhere near where the children are located in the family day home. The provider stated that the numbers are on her refrigerator upstairs.

Plan of Correction: I will bring it down today.
