The inspector discussed the following with the provider: out-of-state forms and information available on the DSS website, rest time, new subsidy requirements, points allowed per caregiver, and model forms available to providers.

An unannounced focused monitoring inspection was conducted today from 10:30am -1:00pm due to the Provisional status of the current license. There were 12 children (41 points) directly supervised by 3 staff. Five children's records, 2 household member?s records, and 3 staff records were inspected. The inspector reviewed the violations from the previous inspection for compliance. Children were observed playing freely, doing worksheets, napping, and eating lunch. Today's lunch consisted of macaroni and cheese, chicken, corn, and bananas. The exit interview was held with the provider at the conclusion of the inspection. Standard #: 22VAC40-111-100-A Description: Based on record review, the family day home did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child#5's record is missing a physical examination, the child's first day of attendance was documented as 12/18/2018. Plan of Correction: We will request a copy of the child's physical.
Standard #: 22VAC40-111-60-B Description: Based on record review, it was determined that children's record were incomplete. Evidence: Child#1's record is missing the address for 1 of 2 emergency contacts. Child#2's record is missing the father's employer, and the employer's telephone number. Plan of Correction: We will request the information from the parents. The father does not have a work telephone number.
