Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the new renewal application that was submitted to the Licensing Inspector on site during today's inspection.

Comments:
The following inspection took place between the hours of 3 pm through 5 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Lenah Falls, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (6 points) with 1 caregiver. A total of 7 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: learning, homework time, snack time, hand washing, bathroom breaks, tutoring, pick-up time, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 7 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (03/06/2019), documentation of a physical examination was not available for review for Child #7. 2. The start date for Child #7 was listed as 01/07/2019.

Plan of Correction: A current physical shall be obtained for Child #7 and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 7 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (03/06/2019), the address for 2 of 2 emergency contacts listed was not available for review. 2. While the name and phone numbers were listed, the address was left blank for both.

Plan of Correction: This missing information shall be obtained and placed on file.

Standard #: 22VAC40-111-90-A
Description: Based on review, 2 of 7 children's files reviewed indicated that before a child may attend the Family Day Home (FDH), the Provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence - 1. On the date of inspection (03/06/2019), a current immunization page was not available for review for: Child #6, and Child #7. 2. Both Child #6, and Child #7 were in attendance during the date of inspection.

Plan of Correction: A current immunization page shall be obtained and placed on file for each child.
