Technical Assistance:
Reminder that 3 children's files need to be reviewed and updated by the parents before the end of the day.

Comments:
An unannounced monitoring inspection was initiated today. The physical plant, 2 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2:50-3:50pm. There were 12 children, totaling 32 points, under the supervision of 2 caregivers. Today the children worked with play-doh, counting numbers, circle time with storytime, rested during naptime and listened to music. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served grilled cheese, chicken noodle soup, peas and carrots, cantaloupe and milk for lunch. The Provider is certified in MAT (medication administration training). Standard #: 22VAC40-111-60-A
Description: Based on file review, 1 out of 5 children's records has not been updated annually. Evidence 1. Child #2's record was last updated on 11/2/17.

Plan of Correction: I will ask the parents to update the file.

Standard #: 22VAC40-111-180-A
Description: Based on file review, 2 caregiver/household members do not have a TB test/screening completed every two years. Evidence 1. Caregiver/household member #1's most recent TB test was completed on 1/3/17. 2. Caregiver/household member #1's most recent TB test was completed on 12/27/16.

Plan of Correction: The Provider stated that these have been completed, but were unable to be located during the inspection. If they are not found by the end of this week, they will contact the dr for a copy to keep on file.
