Standard #: 22VAC40-111-240-A Description: Based on observation, one area of the Family Day Home (FDH), outside, was not maintained in a safe condition. Evidence - 1. On the date of inspection (03/19/2019), the gate to the entrance of the side yard that leads into the playground and then to the back lower entrance of the FDH, has space greater than 3 1/2 inches to 4 inches between each vertical wooden slat. The barrier around the H/C Unit on the side of the house in the playground area also has space greater than 3 1/2 to 7 inches. 2. These barriers may pose a threat as possible head entrapments. 3. The FDH outside playground is used by the children in care. The FDH license can enroll children from birth through 12 years. Plan of Correction: Additional slats shall be added to existing space to narrow these openings to under 3 1/2 inches. Until such time, these areas shall be monitored.
