An unannounced mandated Monitoring inspection was conducted today from 10:00 to 11:30. There were 12 children in care (30 points) with 3 caregivers. Children were observed singing songs, reading books, dancing to music, playing with puzzles and interacting with caregivers. Reviewed 3 staff files, 1 resident file, 2 children's files, attendance, menu, emergency evacuation log and supplies, cribs, first aid kit, and physical plant. Standard #: 22VAC40-111-120-B Description: Based on record review, staff files were incomplete. Staff#3 did not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Hire date 3/15/2019. Plan of Correction: I checked one reference and will check the second.
Standard #: 22VAC40-111-60-A Description: Based on record review, children's records did not have documentation of annual parent review of information. Evidence: Child#1 and Child#2 did not have parent signatures indicating there had been an annual review of emergency contact information. Plan of Correction: I will have the parents update
Standard #: 22VAC40-111-520-C Description: Based on observation, cribs do not meet the current U.S. Consumer Product Safety Commission standards. Evidence: Inspected 6 cribs and 5 were manufactured prior to the 2011 Consumer Product Safety Commissions standards for cribs. Crib dates were 2003, 2009, 2010. Provider had documentation she received from a company indicating the cribs met safety standards, however, the company name on the document (Angeles) did not match the manufacturer of the crib (US Baby). Plan of Correction: I will get a new letter from US Baby and send you a copy.
