Comments:
An unannounced renewal inspection was conducted on March 27, 2019, between the hours of approximately 10:15 a.m. and 1:30 p.m. Ten children were enrolled and eight children were in the care of three caregivers, on this date. Two resident children were also present in the home. Four caregivers records, one household member's record and five children's records were reviewed and/or updated. No medications were on site/administered. The areas of the home used by the children in care and outdoor play areas were toured, available records/documents and caregiver training (first aid/cardiopulmonary resuscitation and medication administration training) were reviewed and safety equipment was inspected/observed (first aid kit, flashlight, radio and fire extinguisher). Posted information (daily schedule, menu, emergency telephone numbers and emergency evacuation plan/diagram) and storage of cleaning supplies/hazardous substances were reviewed/observed. Program observation was completed. Activities provided on this date included: neighborhood walk; diapering/restroom/handwashing procedures; snack; outdoor play and naptime. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The caregivers were observed to be patient and responsive to the individual needs of the children. The daily schedule was consistent with the activities provided. The children knew and followed the daily routine and transitions well. One previously cited violation was reviewed for compliance; no repeat violations were cited. Exceptions to compliance were noted on the accompanying violation notice. Standard #: 22VAC40-111-750-A
Description: Based on observation of topical skin products (diaper and topical ointments) and discussion with caregiver #1 on March 27, 2019, the program failed to ensure one topical ointment, provided by the child's parent, was labeled with the child's name. Evidence: 1. Observation of one topical ointment, on this date, determined that Vaseline for child F was not labeled with the child's name. 2. Caregiver #1 confirmed the lack of labeling.

Plan of Correction: All topical skin products when provided by the children's parents, will be labeled with the children's names.

Standard #: 22VAC40-111-120-B
Description: Based on review/update of four caregiver records and discussion with caregiver #1 on March 27, 2019, the program failed to obtain verification of age for two caregivers. Evidence: 1. Review/update of records for caregivers #2 and #4 determined that verification of age was not on file. 2. Caregiver #1 confirmed the lack of age verification in the employee records.

Plan of Correction: Verification of age will be obtained and included in all caregiver records.
