22VAC40-80 THE LICENSE.
An unannounced monitoring inspection was completed on March 28, 2019 from approximately 10:15a.m. to 11:30a.m. Four children were in attendance, infant to four years old, for a total of ten points. The children were observed interacting with the provider during arrival to the home, group learning activities, group exercise time and infant activities. The indoor physical environment and equipment was inspected. The outside space has not been used due to weather. Five children?s records, the fire drill and shelter in place logs, daily attendance and required postings were reviewed. Transportation has not been provided yet and medication will be administered but there are no enrolled children on medications. There were no injury reports to review. The provider was available for interview, the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the provider. There were two citations for violations of the Standards for Licensed Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.
Standard #: 22VAC40-111-920 Description: Based on observation, the licensee failed to date a menu when meals or snacks are provided by the family day home. Evidence: 1. A review of the week?s current posted menu lacked a date. 2. Caregiver #1 acknowledged the menu was not dated. Plan of Correction: Corrected on site. The provider will ensure she dates the menus.
Standard #: 22VAC40-111-100-B Description: Based on a review of children?s records, the licensee failed to obtain the physical examination within twelve months prior to attendance for one of five child.
Evidence: 1. Child #5, four years of age, was enrolled on January 8, 2019. Child #5's record contained a physical dated October 11, 2017 and therefore not within the required time frame for the child's age. 2. Caregiver #1 acknowledged this physical was not obtained within the required time frame.
Plan of Correction: The provider will obtain an updated physical from the child's parent. The appointment is already scheduled.
