An unannounced monitoring inspection was completed on March 29, 2019, from approximately 1:15p.m. to 2:15p.m. There were no children were in care during the inspection due to home maintenance. The physical environment, children's play equipment and materials, the first aid kit and emergency supplies and nutrition were inspected. The outdoor space is currently not used. Five children?s records, three caregiver records, one household member's record, the emergency preparedness and response plan, fire drill and shelter in place drill logs, daily attendance and required postings were reviewed. There were no injury reports to review. The provider was available for interview, the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were three citations for violations of the Standards for Licensed Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the licensee failed to obtain documentation that the child has been adequately immunized before the child may attend the family day home for two of five children.
Evidence: 1. Child #1's record, enrolled March 12, 2019, contained an immunization record dated March 20, 2019. Child#4's record, enrolled February 26, 2018, contained an immunization record dated March 2, 2019. Therefore neither were obtained before the child attended the family day home. 2. Caregiver #1 acknowledged these immunization records were not obtained by the required time frame.

Plan of Correction: The provider will require that parents submit immunization records prior to start.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, the licensee failed to ensure that each caregiver obtain a tuberculosis screening every two years for one of three caregivers. Evidence: 1. Caregiver #1?s record contained a tuberculosis screening/test dated February 3, 2017 and there was no documentation of a subsequent screening/test. 2. Caregiver #1 acknowledged a subsequent tuberculosis screening/test has not been completed as of the due date of February 3, 2019.

Plan of Correction: Caregiver will obtain a subsequent screening/test. Caregiver will make the appointment the next business day.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver records, the licensee failed to ensure caregiver obtained 16 clock hours of annual training for one of two caregivers.
Evidence: 1. Caregiver #3 was hired on March 19, 2018. Caregiver #3?s record reflected approximately two and a half hours of annual training hours for 2018-2019. 2. Caregiver #1 acknowledged Caregiver #3 has not obtained annual training as required.

Plan of Correction: This cannot be corrected but provider will ensure that all caregivers obtain annual training for next year as required.
