Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and paperwork on file for new assistant.

Comments:
The following inspection took place between the hours of 9 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (32 points) with 2 caregivers. A total of 12 children?s files, 3 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: art, learning, large motor skills outside in the FDH backyard, infant sleeping, diapering, bathroom breaks, hand washing, toy play, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-90-C
Description: Based on review, 3 of 12 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children who were not exempt from the immunization requirements (according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (04/04/2019), the following children's files were without a current immunization record: Child #1, Child #9, and Child #12. 2. The most current immunization records on file were: 09/12/2018 (for Child #1), 09/17/2018 (for Child #9, and 09/25/2018 (for Child #12.

Plan of Correction: Updated immunization records shall be obtained and placed on file for all applicable children.

Standard #: 22VAC40-111-180-A
Description: Based on review, 2 of 12 children's files reviewed indicated that the Provider did not obtain for each adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every two years. Evidence - 1. On the date of inspection (04/04/2019), the most current TB documentation on file for Household Member #1 was dated - 03/29/2017. 2. This review was confirmed by the Provider on the date of inspection.

Plan of Correction: Updated TB documentation shall be obtained for Household Member #1 and placed on file.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview, a furnishing of the Family Day Home (FDH), inside, does not have supporting documentation that states that it meets the new safety standards set by the Federal Government, Consumer Product Safety Commission (CPSC) that went into effect on December 28, 2012. Evidence - 1. On the date of inspection (04/04/2019), it was determined that the date of manufacture placed on a sticker attached to the side of the FDH crib was displayed as June 2009. 2. The Provider stated (during an interview conducted on the date of inspection) that this crib was given to the FDH in order to accommodate a new child.

Plan of Correction: This crib will no longer be used and will be removed. Children older than 12 months shall begin their transition to a mat/cot. Corrected on the date of inspection.
