Comments:
A Subsidy Health and Safety inspection was conducted on 4/12/19 to review supplemental health and safety requirements. There was one child (2 points) in care of 1 caregiver. The child was observed playing with toys. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on staff interview, the family day home did not maintain a continuity plan to be added to the emergency preparedness plan. Evidence: When asked to see the continuity plan, the provider stated that she had not prepared one and it was not added to the emergency preparedness plan.

Plan of Correction: I will have a plan ready and added to the emergency plan.

Standard #: 22VAC40-665-410-A-1
Description: Based on staff interview, the family day home did not practice monthly evacuation drills. Evidence: The provider stated that she had not practiced an evacuation drill for the month of March 2019.

Plan of Correction: I will conduct two fire drills and document in the month of April.
