Comments:
An unannounced subsidy inspection for the religiously exempt child day center was conducted on 04/08/2024. There was a total of 62 children, ages 2 months-10 years and 17 staff present during the inspection. A total of 7 children?s records and 6 staff records were reviewed during the inspection. The inspector arrived at 9:00 am and departed at 11:45 am.

Information gathered during the inspection determined non-compliance with the subsidy requirement and were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-790-520-F
Description: Based on observation and staff interviews, the vendor failed to maintain, in a way that is accessible to all staff who work directly with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions.

Evidence: The record for Child 7 contained documentation of a diagnosed food allergy. There was not a current written list of all children?s allergies accessible to all staff who work directly with children. Staff 7 confirmed that there was not a list of all children?s allergies, sensitivities and dietary restrictions.

Plan of Correction: Has been created and will be placed in each class.

Standard #: 8VAC20-790-540-B-11
Description: Based on record review an staff interviews, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child 7 contained documentation of a diagnosed food allergy. There was not a written allergy care plan for Child 7. Staff 7 confirmed that they did not have an allergy care plan for Child 7.

Plan of Correction: Has been requested.

Standard #: 8VAC20-790-820-B
Description: Based on observations, the vendor failed to ensure that when a child is placed in an infant seat, high chair or feeding table, the protective belt shall be fastened securely.

Evidence: A child in the Infant Class A was observed in an infant seat, the protective belt of the infant seat was not fastened.

Plan of Correction: Reviewed with the staff.
