The provider and the Licensing Inspector discussed the procedure for transferring the license to a new home.
An unannounced monitoring inspection was conducted on April 23 2019 between 10:00am and 11:15am. There were 12 children (24 points) in the direct care of the provider and an assistant. Children were observed creating art projects, working on sentences, completing puzzles, and working together on work. The family day home is clean, organized, and offers plenty of space for the children. There is an abundance of materials for the children to choose from. Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 2 staff records reviewed did not have a current report of Tuberculosis screening every two years from the date of the first screening. Evidence: Staff #2 record last documented report of Tuberculosis screening is dated February 2 2017. Plan of Correction: We will get the screening form.
