An unannounced monitoring inspection was conducted today from 12:00pm-2:45pm. There were 10 children (29 points) in care, including the Provider's own child, in the direct care and supervision of 3 caregivers. There is a separate Nanny for the Provider's child. The first aid kit, 4 children's records, 3 staff/household member records, evacuation drills, emergency materials, and the home/day care areas were reviewed. Standard #: 22VAC40-111-170-A
Description: Based on record review, 1 out of 3 caregiver records did not include documentation of a tuberculosis screening prior to coming in contact with children. Evidence: 1. Caregiver #3 (hire date: 9/10/2018) did not have documentation of a tuberculosis screening during the time of the inspection.

Plan of Correction: She will bring it tomorrow.

Standard #: 22VAC40-111-180-A
Description: Based on record review, documentation of tuberculosis screening reports were not obtained every two years from the date of the most recent screening for 1 out of 3 caregivers. Evidence: 1. The most recent report of tuberculosis screening for caregiver #1 was dated 8/15/2016.

Plan of Correction: I will get an updated one today.
