Equipment in the front yard is for family use only. We also discussed choking pieces with older children's toys.
An unannounced monitoring inspection took place today from 10:45 am - 12:45 pm. There were six children present with the assistant at the beginning of the inspection for 14 points while the provider took older children to school. The provider returned within 20 minutes. I observed children working with puzzles, doing the "bean bag boogie", engaged in dramatic role playing with costumes and eating lunch. Children also made collages of birds nests after learning about birds and reading related books. I also observed a smooth transition between activities. Records were well-organized. Standard #: 22VAC40-111-120-B Description: Based on a review of records today, one item was not included in the assistant's record as required. Evidence: There was only one reference documented, and the full name of that reference was not complete. Plan of Correction: I will get these.
Standard #: 22VAC40-111-70-A Description: Based on a review of six children's files, one did not have all required information. Evidence: The parent of Child # 5 did not sign the form for Notification of Medication Policy, Emergency Procedures, Liability Information and Information for Parents form. Plan of Correction: I will get it signed today.
Standard #: 22VAC40-111-240-A Description: Based on observation of the physical environment, there were some areas and equipment not maintained in a clean, safe and operable condition as required. Evidence: Outdoors: The green plastic slide portion of the climbing structure was not attached with screws, leaving the slide loose. There were sharp and splintery areas in the fence; and a sharp edge where the fence had broken. Nails on the gate were protruding and accessible. A window frame and the broken swing were accessible on the ground. Plan of Correction: We had to get the tree taken down but some branches hit the equipment. We had to remove the swing set. We are not using the playground area until it is fixed. Indoor: Areas of chipping paint will be covered.
