Discussed background checks.
An unannounced monitoring inspection was conducted today from 11:15am-1:30pm. There were 11 children (29 points) supervised by 2 direct care staff. The home, 2 staff records, 6 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside, eating lunch, and transitioning to naptime. Lunch served was rice, mixed vegetables, grapes, carrots, potatoes, and milk. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, 4 of 6 children's records did not contain all of the required documentation. Evidence: 1. Child #1 (start date: 4/2/2019) and Child #3's (start date: 7/9/2018) records did not contain documentation of a parent signature indicating the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. 2. Child #5's (start date: 2/1/2019) record did not contain documentation of a completed physical form signed by a physician. 3. Child #6's (start date: 1/29/2018) record did not contain documentation of parent acknowledgement of an annual review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review of emergency contact information was dated 2/26/2018. Plan of Correction: The parent will bring a copy of the physical and they will sign the children's records today.
Standard #: 22VAC40-111-170-A Description: Based on a review of caregiver records, 1 of 2 caregiver records did not contain documentation of a current report of TB screening prior to coming into contact with children. Evidence: Caregiver B's (date of hire: 12/2018) record did not contain documentation of a current report of TB screening. Plan of Correction: The employee will get the TB test done.
Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver records, the provider did not maintain documentation of completed orientation by the end of their first week of assuming job responsibilities. Evidence: Caregiver B's (date of hire: 12/2018) record did not contain documentation of completed orientation training. Plan of Correction: I will print orientation today and go over everything with the employee.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of caregiver records, 1 of 2 caregiver records did not contain documentation of central registry findings within 30 days of employment. Evidence: Caregiver B's (date of hire: 12/2016) record did not contain documentation of completed central registry findings. Plan of Correction: We will check the email for results.

Evidence: Caregiver B's (date of hire: 12/2018) record did not contain documentation of completed fingerprint results. Plan of Correction: We will make an appointment as soon as possible.
