An unannounced monitoring inspection was conducted today. There are 3 children present (10 points) with 3 caregivers. One child's and 2 resident's records were reviewed. See Violation notice form missing item. Children were observed playing with a wide variety of toys and games. Emergency supplies, emergency evacuation drills, shelter-in-place drills, refrigerator/freezer temperature and first aid supplies are all in compliance. Standard #: 22VAC40-111-60-B Description: Based on a review of records, one child's record is incomplete. Evidence: Child 1's original proof of birth documentation was not viewed. The parent provided a copy of the certificate. Plan of Correction: I will ask the parent to bring in the original certificate.
