Arrived 11:05am Departed 1:30pm A Subsidy Health and Safety inspection was conducted on 05/29/2018 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and is noted on the violation notice. Thank you for your cooperation. Description: Based on a review of documentation, the provider failed to conduct an annual lockdown drill. Evidence: 1. Emergency drill records show that the last lockdown drill was conducted in January of 2018.

Plan of Correction: The provider will ensure that lockdown drills are conducted annually.
