54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted on 5/31/19 from 11:20am until 12:20pm. At the time of entrance, there were 4 children in care, for a total of 9 points, with the provider. A sample of 4 children's records and 2 provider/household member records were reviewed. Children were observed engaging in free play with age appropriate toys and participating in morning learning activities. The provider does not currently administer medication or provide transportation at this time. There have been no injuries to children in care since the last inspection. First aid and emergency supplies, documentation of emergency practice drills, the emergency preparedness plan and required family day home postings were reviewed. Two violations in the areas of administration and physical health of caregivers and household members are identified on the violation notice and were discussed with the provider during the exit interview.
Standard #: 22VAC40-111-60-A Description: Based on record review and interview, the provider did not ensure that an up-to-date record is maintained for each enrolled child. Evidence: 1. The record for child 1 does not contain documentation of proof of the identity and age of the child. 2. The record for child 2 does not contain the child's start date and only contains one emergency contact, where two are required. 3. The provider confirmed that the records for child 1 and child 2 do not contain all the required information. Plan of Correction: The provider stated she will call child 1's parent and have them bring a copy of the child's birth certificate at pick-up today. The provider stated child 2's parents have given notice and the child will no longer be attending the family day home. The provider will document child 2's start date in the record.
Standard #: 22VAC40-111-170-A Description: Based on record review and interview, the provider did not ensure that all household members shall obtain a current Report of Tuberculosis Screening prior to coming into contact with children. Evidence: 1. The record for household member 1 (age 18) does not contain documentation of a current tuberculosis screening. 2. The provider confirmed that household member 1 has not yet obtained a tuberculosis screening since turning 18 years old. Plan of Correction: The provider stated household member 1 will obtain a tuberculosis test or screening next week.
