Technical Assistance:
Annual Staff Training will be reviewed at the next licensing inspection. Staff have obtained some annual training since the last October inspection.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 10 children(25pts) with Provider and 2 assistants were present. When the Inspector arrived, the children were in two groups, one group of children were playing in the play room and the other group was also playing in the crib room. A nice transition was observed from morning play to class time where the children reviewed the day of the week and the weather. They also completed an art project using stickers and construction paper. One group transitioned to outside play, followed by the second group. Children sat down and individually put their shoes on with staff assistance if needed. A total of 5 children's records, all staff and 1 household record was reviewed. Previous violations from the October licensing inspection were reviewed, 2 repeated violations found. This inspection began at approximately 9:45am and ended at 11:30am. Standard #: 22VAC40-111-750-A
Description: Based on review of 5 children's records, parent authorization for topical ointments were not obtained. Evidence: Child # 2, 4 and 5 have topical ointments. Authorizations for these ointments were not found.

Plan of Correction: I will have the authorizations completed by the parents today.

Standard #: 22VAC40-111-960-B
Description: Based on observation of the programming, 1 infant was not held while being bottle fed. Evidence: Child #1 was in a bouncy seat while Staff #3 held their bottle to feed them. Infants who can not hold their own bottle must be held while being fed.

Plan of Correction: We will always hold infants who can not hold their bottles while feeding.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, 3 out of 5 records was not complete. Evidence: Child #1's record did not contain addresses for both parents work address. Child #2's record did not contain a signed written emergency medical authorization for the Provider. Child #4's record did not contain a copy of the Provider's Medication Policy.

Plan of Correction: I will obtain these addresses at pick up. And I will provide my medication policy to the child's parent.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the crib room, one area of the home was not maintained in a safe manner. Evidence: The left corner of the crib room has carpet that has separated from the corner, exposing a carpet tack.

Plan of Correction: Provider blocked this area off but will have this area repaired.

Standard #: 22VAC40-111-250-C
Description: Based on observation of the crib area, items were tied across the top of a crib. Evidence: Child #1 is 7 months old. The designated crib for this child has a toy tied across the top of the crib. Items must be removed from cribs for children over five months of age. This is repeated from the October licensing inspection.

Plan of Correction: I will remove this toy immediately.
