Standard #: 22VAC40-111-770 Description: Based on review of the radio, and interview with the Provider, a battery-operated weather band radio was not available. Evidence: An AM/FM portable radio was available but it wasn't a weather band radio. Plan of Correction: I will order the weather band radio right away.
Standard #: 22VAC40-111-60-A Description: Based on record review, 2 out of 2 children's records did not have up-to-date, completed, information for enrolled children. Evidence: The files for Child #1 and Child #2 were incomplete. Missing: parent's employee addresses, addresses for emergency contacts and authorization to administer prescription or non-prescription medications. Plan of Correction: We will use the forms from DSS and make sure to ask parents to fill out all information.
Standard #: 22VAC40-111-70-A Description: Based on review of records, 2 out of 2 children's files did not have required written information for parents. Evidence: Child #1 and Child #2 were missing documentation regarding Liability Insurance. Plan of Correction: We will use the form from DSS and make sure each child's file has the information.
Standard #: 22VAC40-111-80-B Description: Based on record review, and interview with the provider, the identity and age of children was not documented for verification. Evidence: The Provider stated she reviewed original birth certificates for Child #1 and Child #2 when they first enrolled, but didn't write down the birth certificate numbers or date reviewed. Plan of Correction: I will ask for the originals again and write down the required information.
Standard #: 22VAC40-111-320-G Description: Based on record review, and interview with the provider, the fireplace had not been inspected. Evidence: The Provider stated the fireplace was inoperable but they will get the inspection completed and documentation required. Plastic, a screen and baby gate blocked the fireplace from childcare. Plan of Correction: I will call an inspector today to set up an appointment.
