An unannounced monitoring inspection was conducted June 11 2019 between 10:45am and 12:30pm. There were 6 children in the direct care of the provider and an assistant. Children were observed eating lunch, reading, being diapered, and playing with toys. The home offers age-appropriate materials for the children in care. Standard #: 22VAC40-111-800-B Description: Based on observation, the emergency plan was not reviewed and updated annually or as needed. Evidence: The emergency preparedness plan is documented as last reviewed May 8 2017. Plan of Correction: Will review and sign it today
Standard #: 22VAC40-111-830-A Description: Based on observation, the family day home did not conducted emergency evacuation procedures monthly. Evidence: The evacuation drill record form was last completed February 2019 and does not contain documentation of drills conducted March, April, or May of 2019. Plan of Correction: I will check with the calendar and fill in the documentation.
Standard #: 22VAC40-111-60-B Description: Based on record review 2 of 4 children's records reviewed did not have all of the required information. Evidence: Child #2 record is missing the street number for the emergency contact. Child #3 record is missing the complete address and telephone number of an emergency contact. Plan of Correction: I will give the parents a note today.
Standard #: 22VAC40-111-90-C Description: Based on record review, 1 of 4 children's records reviewed did not have documentation of additional immunizations every 6 months for children under the age of 2 years. Evidence: Child # 4 is under two years of age. The date of the last documented immunization record is January 26 2018 Plan of Correction: I will request a copy from mom.
Standard #: 22VAC40-111-180-A Description: Based on record review, 2 of 2 adult records did not have documentation of a current Report of Tuberculosis Screening every two years from the date of the first screening. Evidence: Staff #1 and Staff# 2 record has last documented Tuberculosis screening dated May 24 2017 Plan of Correction: We will do it next week.
