54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Please submit the renewal application within 30 days of the expiration date of the Conditional License. As discussed, health examinations for children are required within 30 days of enrollment. Discussed staffing requirements during meal preparation as your enrollment increases. Discussed the need to reposition infants every 30 minutes to ensure stimulation and activity.

Comments:
An unannounced non-mandated inspection was conducted this date. A total of 6 children(16pts) were present with the Provider and 1 assistant. When the inspector arrived, the younger children were completing their morning snack. The children were observed in high chairs. The older children were playing at a table with a doll house and some were dressed in princess costumes. Walk thru of the programming area, a review of 5 children's files, observation of the crib area was conducted. Emergency evacuation drills were found current. The transition from morning activities to lunch time was observed. Lunch today was chicken, rice, peas and tomatoes with milk to drink. Hand washing procedures were observed by both staff and children. This inspection began at approximately 10:45am and ended at 1pm. Thank you for your cooperation today. Standard #: 22VAC40-111-750-A
Description: Based on review of topical ointments, one authorization was not complete. Evidence: The topical ointment for diaper cream for Child #1 and #3 did not include the effective dates of authorization.

Plan of Correction: The parent s will complete the authorization form to include the effective date.

Standard #: 22VAC40-111-60-B
Description: Based on review of 5 children's records, 4 out of the 5 records were incomplete. Evidence: Child #1's record does not contain a signed parent acknowledgement. Child #2's record does not contain documentation of provisions of the emergency plan. Child #3 and #4's record does not contain the medical insurance number.

Plan of Correction: All information that is missing will be obtained by the parents.
