54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Consultation was given on increasing capacity as requested on the renewal application. The new capacity can only be allowed with a new certificate of occupancy from Arlington County that permits 6 children. A fan is needed in the living room area if the temperature reached 80 degrees or higher and is allowed as long as a mesh guard is in place.

Comments:
An unannounced renewal study was conducted today from 10:00-11:15. Four children were present with two caregivers for twelve points which is within ratios for supervision. The home has two rooms for day care purposes. Observed children engaged in free play, singing, blocks and reading stories as well as infant care. Two children's records as well as all household members andcaregivers were reviewed. Emergency preparation is current. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-940-A
Description: Based on observation, the temperature in the refrigerator is above 40 degrees. Evidence: The thermometer registered 51 degrees during the inspection.

Plan of Correction: The temperature control was lowered to make it colder during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 0ne of two children's records is incomplete. Evidence: Child #1 is missing an emergency contact address and the insurance policy number.

Plan of Correction: The provider will request the information from the parent and complete the record.

Standard #: 22VAC40-111-320-B
Description: Based on observation, an outlet was missing a protective cover. Evidence: The outlet by the dining room table was missing the child resistant protective cover.

Plan of Correction: The cover was replaced during the inspection.

Standard #: 22VAC40-111-540-B
Description: Based on observation, an prohibited piece of equipment was in use. Evidence: Child #2 was observed in an infant walker upon arrival to the family day home.

Plan of Correction: The walker will be removed as of the inspection.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper pad is no longer non-absorbent. Evidence: A small slit is seen in the diaper pad.

Plan of Correction: The pad will be replaced.
